Linguistically, although my immediate reaction to "fuck" (noun) used as an insult is similar to sairaali, I take your point that it can probably be either fucker or fuckee, but in my culture (British English) the overwhelming majority of "fuck" type sexual insults are aimed at fuckees, i.e. women and homosexual men, and even when they're aimed at heterosexual men they're generally implying he's not doing enough fucking to be A Real Het Man (e.g. "jerk"). /personal observation
